Please take a minute to print and fill out the new patient forms below before your first appointment and bring the forms with you.

If an adolescent/minor patient will not be accompanied by a parent and/or someone else will be bringing them to their first appointment, please fill out our permission for treatment form and have the patient bring the form with them to their first appointment.
